Repeat Prescriptions
You may be able to request a repeat prescription without attending the doctor. However if the Doctor needs to see you we will contact you in due course.
WE ARE NOW SENDING PRESCRIPTIONS DIRECTLY TO YOUR PHARMACY.
PLEASE SPECIFY THE PHARMACY YOU WANT TO COLLECT FROM ON THE FORM.
There is an administration charge of €25 to issue a prescription. We will contact you to arrange payment of this charge over the phone.
To avoid errors we regret that prescriptions cannot be ordered over the telephone. Please complete the form below at least 48 hours before your prescription is due for renewal.
The prescription will be available for collection from your DESIGNATED PHARMACY within 5 WORKING DAYS. Please make allowances for weekends and public holidays.
